Incision Placement Options
Many factors affect the placement of the incision in breast augmentation surgery, such as breast implant type, degree of augmentation, and your particular anatomy. Regardless of which incision site is used, the goal is always to minimize visible scarring. Dr. Romanelli will take into consideration your personal circumstances to help you determine which of the following options is appropriate for you.
- A periareolar incision is made along the bottom border of the areola - the more darkly pigmented circle of skin surrounding the nipple. Although the incision is directly on the breast, in most cases this method will create very little noticeable scarring while providing direct access to the interior breast anatomy.
- An inframammary incision is chosen by the majority of Dr. Romanelli's patients. This incision is made just below the breast where it connects with the chest wall. This approach provides the surgeon the most direct route for implant placement and adjustment. It is hidden within or slightly above the fold of the breast.
- A transaxillary incision is made in the crease of the underarm, so scarring is not very visible. However, this incision is not hidden beneath a bra or bikini top, and the incision generally cannot be used again for secondary surgeries.
Implant Size Choices
Choosing the right size implants can seem intimidating, but Dr. Romanelli and his nurses help make the process comfortable and reliable. There is no absolute "right size" but instead there is a range of sizes that will look proportional and attractive on a woman's figure. Dr. Romanelli takes specific breast measurements in order to precisely identify nipple position, breast asymmetries, and, most importantly, the optimal width for each patient's breast implants. Then the patient puts on a surgical bra and "tries out" her new breast fullness using breast forms or "sizers." Implant Placement Options
Breast implants may be inserted either under the pectoral muscle (submuscular or subpectoral placement) or over the pectoral muscle (subglandular). - Submuscular placement positions the implant behind the pectoralis (chest) muscle. Patients may experience a slightly longer, less comfortable recovery compared to the subglandular method. There is less interference with mammograms with submuscular implants. Dr. Romanelli recommends this placement for more than 95% of his patients.
- The subglandular method places the implant directly behind the breast tissue, but above the pectoral muscle. Because there is no cutting of the muscle, recovery is usually more comfortable. Dr. Romanelli may recommend for women with some specific breast shapes. It is sometimes an option for women who may otherwise need a breast lift, but who wish to avoid the scars or a lift. However, this placement method may make implants appear more noticeable, particularly for women with small breasts or thin skin.
Anesthesia

To that end, anesthesia is always administered by a highly qualified, board-certified M.D. anesthesiologist.
